OBTAINING A LETTER OF RECOMMENDATION

If you are applying to Emergency Medicine, you will need a letter of recommendation know as a SLOE or “Standard Letter of Evaluation.” These letters are written by the Clerkship Director’s team and are based on your performance during the rotation. To remain as objective as possible, your letters will be based largely upon your shift evaluation cards.

REQUIRED DOCUMENTS

In order to request an evaluation you will need to submit the following documents:

Completed CV

Completed Personal Statement

A copy of your official ERAS form

Optional: a photograph

HOW TO SUBMIT DOCUMENTS

At the end of your rotation you can request a SLOE by uploading your documents HERE. Please make sure all documents are in .pdf format. No single file may exceed 1MB. Please allow four weeks for your SLOE to be completed.
